efer back to the interview and evaluation you conducted in the Topic 2 Family Health Assessment assignment. Identify the social determinates of health (SDOH) contributing to the family’s health status. In a 750-1,000-word paper, create a plan of action to incorporate health promotion strategies for this family. Include the following:

Describe the SDOH that affect the family health status. What is the impact of these SDOH on the family? Discuss why these factors are prevalent for this family.
Based on the information gathered through the family health assessment, recommend age-appropriate screenings for each family member. Provide support and rationale for your suggestions.
Choose a health model to assist in creating a plan of action. Describe the model selected. Discuss the reasons why this health model is the best choice for this family. Provide rationale for your reasoning.
Using the model, outline the steps for a family-centered health promotion. Include strategies for communication.Running Head: Family Health Assessment

Analysis of the Findings
Introduction
Family health assessments are essential, for they unveil crucial details regarding functional
health patterns among the members. The findings allow the assessor to determine strengths and
health barriers that pose a threat to the overall well-being of the individuals. Karaca (2016) holds
that the functional health patterns by Marjory Gordon provide a resourceful framework for
establishing the strengths and health challenges. In my research, the responses from an interview
with a close friend’s family will help me understand the health patterns and problems in the
family.
The Family Structure
In my established family, I interviewed Smith’s family. Martin Smith is 59 years old, his wife,
Mary, is 55 years, and they have two children, Anna, 20, and Mitch of 13 years. The names I
have presented are fictitious for confidentiality purposes. The father is an IT analyst, and the

wife is an independent nurse practitioner. Anna is in college pursuing media relations, and their
son is in middle school. The members view the father as the stronghold of the unit. Although
Martin plays a massive role in financing the family’s needs, his wife is supportive. Smith’s family
is African American in the middle class. They hold firmly to their Lutheranism belief in
conveying love and care to community members by engaging in initiatives like feeding the poor.
They live in a mortgage three-bedroom bungalow located in a serene neighborhood in the
vicinity of their children’s educational institutions.
Family Health and Health Behaviors
From my findings, I discovered that family members love swimming. Their home has a pool, and
this enables them to enjoy their hobby. Further, they have sectioned part of their garage into a
small fitness area, which has a treadmill, pull up bars, yoga mats, jump ropes, various weights,
and a stereo system. Based on the nursing background of Mary, the members prefer taking
homemade food, which is healthy and affordable. She makes sure that the meals are nutritionally
balanced. There are no health-risks in the family regarding alcoholism.
Nevertheless, the mother recently diagnosed with headache disorder and musculoskeletal health
problems due to work-related stress and tedious roles like lifting patients. Their son also revealed
having a bullying encounter at school, and this affected his emotional well-being. Due to the
torment, he became a victim of drug abuse. The family has, however, taken steps in Mitch’s
recovery process. Health professionals have also intervened in providing medication and
counseling to help the members.
Health patterns strengths and health problems or barriers to health

Further, through the findings, it is evident that a substantial advantage in this family is their
emphasis on staying fit by religiously sticking to engaging in physical exercises. This may be a
hobby; however, swimming and aerobics help in improving the overall well-being (Bronikowski
et al., 2016). Additionally, their healthy eating pattern through homemade food that follows the
nutritional recommendations is significant health pattern strength. With the nursing skills that the
mother has, she strives to have a healthy family in the meals she prepares. However, the health
challenges that their mother has is a problem that prevents her from doing strenuous jobs. Mary’s
health problem affects the family, and this triggers stress for all the members. Therefore, stress
harms the emotional health of each of the family members. Moreover, the drug abuse situation
with Mitch has destabilized his health, thus affecting his social well-being, emotional, and
physical outlook. Generally, the three health problems are a challenge to the family.
Application of Family Systems Theory
To solicit changes in Smith’s family, the application of the family systems theoretical framework
would influence positive changes. As such, the theory suggests that the interactional patterns in a
family and the functions members play helps in determining behavioral issues, whether
problematic or not (Johnson, & Ray, 2016). With that in mind, a logical solution to lowering the
musculoskeletal disorder and headaches in Mary would be by relieving some home duties to the
children. This would increase her relaxing time and also help the children bond more when
helping out. Also, the creation of more family-time would enhance the emotional bonding where
they all get to talk about their challenges, thus reducing stress-related issues.

Conclusion
In conclusion, understanding the health behaviors in families is crucial, for it creates ground for
establishing factors that contribute to affecting the individual’s well-being. As seen in Smith’s
family, physical fitness, eating balanced diets, and the connectedness enhances their wellness.
Despite there being a few factors like stress and strenuous jobs that act as barriers to healthiness,
the family’s strengths are substantial.
